Root Cause Corrective Action (RCCA) is the structured process of identifying the true underlying cause of a problem — not just its symptoms — and putting in place corrective actions that prevent recurrence, then verifying they worked.

It draws on tools like the 5 Whys, fishbone diagrams, 8D, and fault-tree analysis. The hallmark of good RCCA is that the same problem never comes back.
